An unusual cause of bleeding from the duodenum: pyogenic granuloma.

چکیده
A 65-year-old man was referred to our hospital because of melena and chest pain due to anemia. Medical history disclosed type 1 diabetes mellitus, an acute coronary syndrome, pacemaker implantation because of a sick sinus syndrome and an occlusion of the superior temporal artery. The patient used a coumarin derivative. During admission, the patient had large volume melena and became hemodynamically unstable, requiring erythrocyte transfusion. After stabilization and correction of coagulation, an upper gastrointestinal endoscopy was performed, which revealed an oozing 10-mm diameter strawberry-like polypoid lesion in the second part of the duodenum near the major papilla (Figs. 1, 2). After reaching haemostasis through epinephrin injection, a biopsy was taken. The histological examination (Fig. 3) showed proliferation of capillaries (A) with a mononuclear cell infiltrate (B) in the lamina propria, consistent with a pyogenic granuloma (C: crypt, remnant of duodenal mucosa) (H&E x200). A pyogenic granuloma, i.e. a lobular capillary hemangioma is a benign vascular tumor, usually arising on the skin and in the oral cavity. The exact etiology is unknown, but possible triggers include mechanical irritation and pregnancy [1]. In the gastrointestinal tract, a pyogenic granuloma is a rare condition. The commonest sites are the small intestine, esophagus and colon, but they can occur throughout the entire gastrointestinal tract. In the literature, we found two other cases of a pyogenic IMAGE OF THE ISSUE

Pyogenic granuloma in the small intestine: a rare cause of obscure gastrointestinal bleeding.
A 65-year-old man with a history of cerebral infarctionwas referred to our hospital for investigations to find the cause of recurring melena, which he had been experiencing for 2 months. At a previous hospital, he had had an upper gastrointestinal endoscopy and a colonoscopy that failed to show a cause for his bleeding, and an abdominal computed tomography (CT) scan, which detected a strongly e...
متن کاملPyogenic granuloma on the lower lip: An unusual location
Pyogenic granuloma is a lesion characterized by excessive production of granulation tissue in response to trauma or chronic irritating stimulation. This pathological entity affects more often children and young adult females, being found mainly in the gum. Clinically, the lesions appear as smooth or lobulated red nodules with easy bleeding, occasionally ulcerated. The present work aims to repor...
متن کاملPyogenic granuloma following treatment with pulsed dye Laser: Report of two cases
Pulsed dye laser (PDL) is an effective treatment for port- wine stain and can cause some short- term and long- term side- effects. Pyogenic granoluma is one of its late- onset adverse effects. Herein, we present two cases of pyogenic granoluma following treatment of port- wine stain with PDL.

Background: Pyogenic granuloma is a benign vascular proliferation that is often related to minor trauma by the patient. Objective: Evaluation of the efficacy of cryotherapy in the treatment of pyogenic granuloma. Patients and Methods: Hundred and forty-six patients with pyogenic granuloma were treated by cryosurgery using liquid nitrogen and cotton tip applicator in an open clinical trial. This...
متن کاملExtragingival Pyogenic Granuloma: an Unusual Clinical Presentation
Pyogenic granuloma is thought to represent an exuberant tissue reaction to local irritation. It occurs in second decade of life in young females. Clinically, oral pyogenic granuloma is a smooth or lobulated exophytic growth, pedunculated or sessile, which usually bleeds on provocation. Oral pyogenic granuloma preferentially affects the gingiva. On rare occasion, it can be found extragingivally ...
